How do public forms work?

The public form feature lets you create and share customizable public documents from templates. These templates can be shared as direct links, embedded on website, or displayed as customizable buttons on websites. Clients can view and submit responses based on your configuration.


Creating a public form

You can generate a public document from a template, allowing others to view, fill it out, and submit it.

  1. Navigate to the Templates section.

  2. Hover over the template you’d like to use and click the three dots (⋯) to view more actions.

  3. Select "Share" from the dropdown menu.

  4. Set the Configure verification option.

    Never: No verification code is sent. Selecting 'Never' may allow unverified users to overwrite client data if they use an existing client's email address.

    Existing: A verification code is sent to existing clients only.

    Always: A verification code is sent to all clients, including non-existing ones.

  5. Configure customization options.

    Apply Workspace Branding (Requires Advanced subscription)

    Remove "Powered by Carepatron" Footer (Requires Advanced subscription)

  6. Select a sharing format.

    Direct link: Generate a shareable public link to the template.

    Inline embed: Obtain an embed code to integrate the template directly into a website.

    Button embed: Customize the embed button display and retrieve an embed code for an actionable button on a website.


Viewing public forms

There are two ways for clients to access public forms, depending on the form's purpose.

1. Read-only public forms

  • You can create a read-only document from a template.

  • Read-only templates do not include client information fields or submission options.

  • These templates are for informational purposes only.

2. Submittable public forms

  • You can create a submittable document from a template.

  • These templates include input fields for client information and a submission button.

  • Clients can fill out and submit responses as needed. Additionally, when a client submits a Public Form, the system syncs their data with their existing Carepatron profile if they already have one. If the client is new, a Carepatron client record is automatically created.
