Quickly upload and customize your existing forms or documents for use in Carepatron. Once imported, templates can be edited, shared, and assigned to clients, helping you streamline your workflow.
Here's how to:
Navigate to the Templates Tab.
Select Import.
Choose the Click to upload option, or Drag and Drop the document.
Upload a PDF. Make sure your form has clearly labeled fields, as this will help with mapping.
Customize the template. Once imported, review the structure. You can edit field names, add/remove sections, and format the layout as needed.
Save and publish.

Use clearly structured and labeled forms.
Prefer typed forms over scanned handwritten ones for better accuracy.
Review the imported version to ensure all fields map correctly.
