Can I score assessments in Carepatron?

Scoring automatically calculates scores in real-time as a form is filled out. No manual tallying required!

What is a scoring assessment?

A scoring assessment is a form that automatically calculates a numerical score as questions are answered. It is designed for clinical assessments like the GAD-7, PHQ-9, and AUDIT.

Previously, practitioners had to calculate scores manually after collecting form responses. Scoring automates this entirely.


What is a scoring group?

A scoring group is a field type that acts as a score calculator. Rather than containing questions directly, it links to single-choice questions elsewhere in the form and automatically aggregates their scores. Currently, only single-choice questions are supported

  • One form can have multiple scoring groups (useful for assessments with subscales)

  • Any existing single-choice question can be linked to a scoring group

  • You can build custom assessments beyond the standard templates

Each single-choice question can be linked to a scoring group. When linked:

  • Each answer option gets a score value (auto-assigned as max + 1, or manually set)

  • Score values appear next to each option in edit mode with increment/decrement controls

  • The linked group name is shown below the question


Scoring methods

We have three scoring methods that you can use:

  1. Sum: Adds up linked question scores.

  2. Average: Calculates the mean of all linked question scores.

  3. Custom: Uses sum or average, then lets you apply sequential operations (+, −, ×, ÷) for custom formulas

Note: For a custom method, operations are evaluated left-to-right — for example, sum × 2 + 5 means (sum × 2) + 5. If division by zero occurs, the result is undefined (no score is assigned).


Scoring group visibility

As a client or practitioner fills out the form, the score updates instantly. No need to submit first; the score recalculates on every answer change.

Scores are visible to practitioners in view/respond modes.

The score is displayed in a preview box within the scoring group field, showing:

  • Title and description of the scoring group (when provided)

  • The computed numeric score (bold, highlighted when active)

2. Client visibility

Scoring Groups are invisible to clients. When a client fills out an intake form, they only see the questions. The scoring group and computed score are hidden.

How does scoring assessment work?

To create scored assessments:

  1. Go to Templates.

  2. Click +New Template.

  3. When the template builder opens, click +Add form field.

  4. Select the Scoring group from the options.

  5. Add the title, description, and the scoring method.

  6. Add single-choice questions.

  7. Link each single-choice question to the scoring group. Score values are auto-assigned to options.

  8. Adjust individual option scores as needed (e.g., reverse scoring).

  9. For custom formulas, add operations.

  10. Click Save. The template is automatically flagged as a scoring template.

Note: You can add single-choice questions first, then add the scoring group later.


How to use scored assessments in a note

  1. Go to Clients.

  2. Select the client from the list.

  3. Click New note.

  4. Fill it in during the session.

  5. As each single-choice question is answered, the scoring group recalculates.

  6. Score is persisted to the note form response on save.

Note: We are currently updating our native app, so for now, it is best to use this feature using the web browser version.


FAQs

Can clients see their scores?

No. Scoring groups are always hidden from clients. Clients only see the questions. Only practitioners can see the computed score.

Can I have more than one scoring group in a form?

Yes. Each scoring group works independently with its own linked questions and scoring method. This is useful for assessments with multiple subscales.

Can a question be linked to more than one scoring group?

No. Each single-choice question can only be linked to one scoring group at a time.

What happens if a client skips a question?

Only answered questions contribute to the score. If no linked questions are answered, no score is shown.

Which field types support scoring?

Only single-choice fields can be linked to a scoring group. Dropdown, Yes/No, and other field types do not currently support scoring.
