Carepatron offers a structured Allergies block for capturing allergy and intolerance information in clinical notes, public forms, and the client intake application.
Every entry is stored against the client's record and surfaces in a dedicated panel on the client overview, giving the whole care team a single, reliable view of a client's allergies in real time.

Where allergies can be captured
The Allergies block works the same way across all three surfaces. Every entry, regardless of where it is captured, is stored against the client's record and appears in the client overview panel in real time.
Clinical note: Added via the slash command menu by a clinician during or after a session
Public form: Added to a form and completed by the client in the responder view
Intake application: Presented to the client as part of their intake, before or during onboarding
Allergy entry fields
Each allergy is documented in a two-step guided format. The user picks a category first, then the full set of fields expands. Category, substance, and criticality are required on every entry.
Category: Food, Medication, Environment, or Biologic
Substance: Specific allergen selected from a terminology-backed search (e.g., Peanut, Penicillin)
Criticality: Unable to assess, Low, or High
Reaction: Clinical reaction selected from a terminology-backed search
Condition type: Whether the entry is an Allergy or an Intolerance
Status: Active or Inactive, toggleable inline on the overview panel
Onset date: When the allergy or intolerance began, when known
Terminology-backed fields: Substance and Reaction are powered by a clinical terminology server. This keeps entries consistent and clinically meaningful across every surface where allergies are captured.
Documenting allergies in a clinical note
The Allergies block is available in the note's slash command menu under the Form blocks group.
Open a client's clinical note and type / to open the slash command menu. You can also click +Basic Blocks option.
Set to Responder mode.
Select Allergies from the Form blocks group. The block is inserted into the note body.
Choose a category for the first allergy entry. The full set of fields expands.
Complete the substance, criticality, reaction, and any additional details.
Click + Add allergy to add further entries, or check No known allergies if none apply.
Set to editor.
Complete the note.
Every entry is written to the client's record and appears in the Allergy or intolerance panel on the client overview.
Collecting allergies via a public form or intake application
The Allergies block can be added to any public form or intake application. Clients complete it in the responder view on their own device.
Add the Allergies block to a public form or intake application in the form builder.
Share the form or intake application with the client.
The client sees the Allergies block in the responder view.
They choose a category, complete the fields, and use + Add allergy to add further entries. There is no upper limit on entries.
The client submits the form. Every entry is written to their record and appears on the client overview immediately.
Managing allergies on the client overview
The Allergy or intolerance panel on the client overview lists every allergy recorded for the client, regardless of where it was captured. Entries are grouped under Active and Inactive tabs and are updated in real time.
Add a new allergy: Click New allergy to open a side panel and record an allergy directly from the overview.
Edit an entry: Click any entry to open the side panel in edit mode and update its details.
Toggle Active or Inactive: Change the status inline from the entry without opening the side panel.
Delete an entry: Use the overflow menu on any entry to delete it.
FAQs
Do I need to set anything up to use the Allergies block?
No. The block is automatically available in the slash command menu, in public forms, and in the intake application. The client overview panel is also live on every client record with no configuration required.
What fields are required on every allergy entry?
Category, substance, and criticality are required. Reaction, condition type, status, onset date, and additional details are optional.
Can a client record their own allergies?
Yes. Clients can add and remove allergy entries when completing a public form or intake application in the responder view. Their entries are recorded with the client as the asserter.
Can the same client have allergies recorded from different sources?
Yes. Entries captured in notes, forms, and intake applications all appear in the same Allergy or intolerance panel on the client overview, regardless of where they were recorded.
